What is a Corrective Action Request (CAR)?

A Corrective Action Request (CAR) is a formal notice from the Supplier Quality Control (SQM), often provided during an audit of the business system, requesting the removal of an item, product, process, or non-compliance to create a remedy. Corrective Action Requests demand that the cause of non-compliance of a product, process, or service be removed to prevent recurrence and is usually decided during quality assurance activities. Such non-compliances are handled by corrective action.

Levels of Corrective Action Requests

The DoD has four tiers of Corrective Action Requests, each depending on the nature of the non-compliance:

Level 1

Ordered for non-compliance that is not perceived to be systemic, significant, or recurrent and can be corrected immediately with no further corrective action is required.

Level 2

Issued when non-compliance with the contract cannot be resolved immediately and are provided at the supplier management level responsible for taking corrective measures where the non-compliance with the agreement cannot be corrected.

Level 3

Issued to the supplier’s management to draw attention to the significant non-compliance of the contract and contract solutions such as reductions in payments or denial of business management systems may be involved.

Level 4

Provided to the top management of the supplier when Level 3 has been unsuccessful and may be combined with contractual remedies to include suspension of progress payments, suspension of product acceptance activities. Can also be issued if the contractual non-compliance is severe enough to justify contractual remedies.
